Description

As an Occupational Therapist at Jukebox Health, you will conduct in-home assessments and functional evaluations to deliver personalized home modification recommendations for older adults and individuals with disabilities. You'll collaborate with clients and families to promote fall prevention and safe mobility, working independently while supported by a remote team.

Requirements

• Degree in Occupational Therapy from accredited program
• 2+ years clinical experience
• Experience in home health, aging in place, or disability services preferred
• Strong interpersonal and clinical reasoning skills
• Reliable transportation and valid driver's license

Responsibilities

• Complete onboarding and credentialing
• Conduct in-home assessments and functional evaluations
• Deliver evidence-based home modification recommendations
• Educate clients and caregivers on fall prevention and safe mobility
• Maintain accurate documentation via electronic system
