Description

You will provide after-hours hospice care to patients in their homes, conducting assessments, managing pain and symptoms, administering medications, and supporting families. Your work ensures patients receive compassionate end-of-life care with dignity.

Requirements

• Bachelor's or Associate's degree in nursing or related field
• Active nursing license in good standing in Ohio
• Active CPR certification
• 2-5 years experience in healthcare management, hospice, or palliative care preferred
• Knowledge of hospice care and interdisciplinary care coordination

Responsibilities

• Conduct initial and comprehensive patient assessments to determine hospice needs
• Develop and revise care plans in collaboration with patients and families
• Administer medications and treatments as prescribed by physicians
• Coordinate patient care after hours and support case managers
• Maintain accurate clinical notes and communicate with physicians
