🎯 What You'll Do

• Perform outpatient therapy evaluations to develop care plans
• Utilize research-based approaches for functional independence
• Coordinate care transitions from home health to outpatient
• Educate clients and caregivers on care plans

📋 Requirements

• Current Vermont Occupational Therapist license
• Minimum one year of recent Occupational Therapy experience
• Graduation from an accredited occupational therapy program
