Description

As a home care Physical Therapist, you will conduct home visits to provide rehabilitative care and skilled nursing to adult and geriatric clients, helping them remain at home. You'll assess mobility, develop care plans, and educate clients and families.

Requirements

• Current Maryland Physical Therapist license
• Minimum 1 year recent work experience as a Physical Therapist
• Graduation from an APTA or Committee on Allied Health Education accredited program

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, range of motion, strength, balance)
• Implement interventions for wound prevention and repair
• Develop and update individualized care plans
• Educate clients and family members on rehabilitative care
