💼 About This Role

You'll lead a pod providing community-based care to vulnerable seniors in WelbeHealth's value-based PACE program. You'll manage a smaller patient panel, enabling deeper relationships and seeing the impact of your care daily.

🎯 What You'll Do

• Perform comprehensive in-person assessments for each participant
• Lead and partner with interdisciplinary team to develop care plans
• Coordinate discharge for acute, short-term, and long-term placement
• Provide on-call coverage by phone a few nights or weekends per month

📋 Requirements

• Successful completion of residency program accredited by ACGME
• Master's or doctorate degree
• Active state physician license without restrictions
• State CDS certification and DEA registration

• Board certification
• CPR and First Aid certification upon hire
• Experience in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics
