You'll manage a diverse caseload, providing comprehensive care coordination for patients in Central Massachusetts. 🎯 What You'll Do

• Manage a diverse caseload ensuring quality patient care and coordination
• Develop and implement individualized care plans based on patient assessments
• Coordinate with PCPs, therapy providers, and internal support teams
• Maintain accurate and timely electronic charting with 24-hour turnaround

📋 Requirements

• Associate's or Baccalaureate degree in Nursing
• Current Registered Nurse license in Massachusetts
• 2 years of experience as an RN in a clinical setting
• Reliable transportation, valid driver's license, and auto insurance

✨ Nice to Have

• 1 year of home health experience
• Case management or supervisor experience
• Knowledge of Medicaid paid family caregiving program
