You'll conduct in-home health risk assessments for members in Lucas County, Ohio, as part of Porter's consumer-centered healthcare ecosystem. Your assessments will directly shape care management plans and improve treatment outcomes.

🎯 What You'll Do

• Visit members to conduct in-home health risk assessments
• Evaluate past medical history and review systems
• Perform medication reconciliation and vital signs
• Complete comprehensive physical exams and point-of-care testing

📋 Requirements

• Valid unencumbered Ohio NP license
• 3 years of patient care experience
• Strong sense of empathy and compassion
• Exceptional communication skills
