Description

You will provide home health occupational therapy services to adult and geriatric patients, conducting evaluations, developing individualized care plans, and educating patients and families to promote independent living. Using a tablet for documentation, you'll collaborate with a multi-disciplinary team to deliver high-quality rehabilitative care at home.

Requirements

• Current AZ Occupational Therapist license
• Minimum one year recent work experience as an Occupational Therapist
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time effectively
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations of functional status and ADL/IADL performance
• Develop and revise individualized intervention plans
• Evaluate home environment for hazards and identify equipment needs
• Document patient care interventions and evaluations using an electronic tablet
