You'll perform outpatient therapy evaluations for adult and geriatric patients in senior living communities. You'll maximize functional independence through research-based care plans as part of a multi-disciplinary team.

🎯 What You'll Do

• Perform outpatient therapy evaluations and develop care plans
• Operate at top of licensure using research-based approaches
• Transition care from home health to outpatient services
• Educate clients, family, and caregivers on care plans

📋 Requirements

• Current NJ Occupational Therapist license
• Minimum one year of recent OT work experience
• Graduation from an accredited OT program
