Description

You will provide one-on-one home health visits to adult and geriatric patients in the Thomasville, Archdale, Trinity, and Sophia areas. Your role includes assessments, admissions, medication administration, wound care, and coordination of care, supported by a flexible schedule and touch pad tablet for documentation.

Requirements

• Current RN license in North Carolina
• Minimum one year recent clinical (medical/surgical) experience
• Graduation from accredited nursing program
• Ability to work independently and manage time
• Valid driver's license and ability to travel to cases

Responsibilities

• Follow designated care plan per patient needs
• Perform home visits including medication administration, wound care, treatments, and procedures
• Monitor client conditions and report changes to clinical manager
• Perform client assessments and case management
• Accurately document observations and care using electronic touch pad tablet
