Description

You will conduct outpatient physical therapy visits within senior living communities, focusing on assessing and treating older adults to optimize function and independence through evidence-based interventions. You'll be part of a multidisciplinary team helping clients age in place.

Requirements

• One year prior clinical experience as a licensed Physical Therapist
• Licensed Physical Therapist (PT)
• Experience with senior living communities preferred
• Proficiency in diagnostic testing and care plan development
• Ability to travel to multiple community locations

Responsibilities

• Perform diagnostic tests and measurements for mobility, range of motion, strength, balance, and safety
• Develop and implement individualized care plans with manual therapy, gait training, and balance interventions
• Continually assess and revise the physical therapy plan of care; participate in multidisciplinary meetings
• Educate clients, family members, and community staff on therapy activities to promote health and independence
• Accurately document observations, interventions, and evaluations using a touch pad tablet
