🎯 What You'll Do

• Maintain caseload using evidence-based approaches to promote engagement
• Conduct periodic telephonic and SMS outreach for timely follow-up
• Collaborate with care team on case reviews and delegated tasks
• Coordinate community connections and manage referrals

📋 Requirements

• 1+ years of general work experience, case management preferred
• Bilingual English/Spanish fluency
• Reliable transportation and valid driver's license for field ops
• Hipaa-compliant internet-connected workspace for virtual ops
