💼 About This Role

You'll evaluate and treat patients in their homes, developing care plans and supervising PTAs to help patients regain independence. You'll restore function to prevent disability and help patients achieve maximum performance within their capabilities.

🎯 What You'll Do

• Perform in-home evaluations and develop care plans.
• Supervise 2 Physical Therapy Assistants (PTAs).
• Help patients with mobility and independence.
• Coordinate with physicians and report patient progress.

📋 Requirements

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources

• 1+ year supervising PTAs in home health
• Experience with high acuity patients
• Case Management fundamentals
