Description

You will provide basic nursing care by assessing residents, implementing care plans, administering medications and treatments, and communicating changes to the interdisciplinary team. Your role ensures safe, timely care in a supportive environment.

Requirements

• Current LPN license in state of employment
• CPR and AED certification (or obtain within 6 months)
• IV certification (or obtain within 6 months)
• Long-term care experience preferred
• Ability to stand/walk for long periods and lift heavy objects/transfer patients

Responsibilities

• Assist in assessing residents on admission and ongoing
• Participate in care planning and update care plans as needed
• Administer medications, treatments, and IV fluids (if certified)
• Document assessments and interventions accurately and timely
• Monitor residents for changes in condition and notify physician using SBARR
