🎯 What You'll Do

• Act as liaison between participants and interdisciplinary care team
• Assist with participant experience projects and satisfaction surveys
• Facilitate communication among service providers, families, and caregivers
• Support grievance process and conduct check-ins for at-risk participants

📋 Requirements

• High school diploma or equivalency
• Excellent organizational, customer service, and communication skills
• Ability to present information to various groups
• Must obtain CPR and First Aid certification within 180 days
