💼 About This Role

You'll provide home health visits to clients in Cumberland, Gloucester, and Salem Counties as part of a multi-disciplinary team. Your core impact is enabling clients to receive rehabilitative care at home through assessments and individualized care plans. 🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility and function.
• Develop and implement individualized care plans and interventions.
• Educate clients and family on rehabilitative care and safety.

📋 Requirements

• Current New Jersey Physical Therapist license.
• Minimum one year of recent work experience as a Physical Therapist.
• Graduation from an APTA-approved program.
• Solid computer skills; experience with electronic medical records preferred.
