💼 About This Role

You'll join an interdisciplinary team to deliver restorative and rehabilitative physical therapy to seniors, maximizing their independence and safety. You'll build long-term relationships with participants rather than short-term rehab stints, benefiting from lower patient volumes and work/life balance. This role stands out for its collaborative, holistic care model.

🎯 What You'll Do

• Conduct Physical Therapy assessments in facility and occasionally in home.
• Participate in IDT meetings to develop participant care plans.
• Deliver and document interventions per participant care plans.
• Supervise Therapy Assistants and train staff, participants, and caregivers.

📋 Requirements

• Bachelor's degree in physical therapy
• Valid California Physical Therapy License or eligibility
• Two years experience with frail or elderly population preferred
• CPR and First Aid certification within 180 days of hire

✨ Nice to Have

• Master's or doctoral degree
• Outpatient, acute, inpatient, or skilled nursing experience
• CPR and First Aid certification upon hire
