Description

You will provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes, conducting evaluations, developing individualized intervention plans, and educating patients and families to promote health and independent living.

Requirements

• Current Massachusetts Occupational Therapist license.
• Prior home care experience.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients' functional status, including ADL/IADL performance.
• Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans.
• Evaluate home environment for hazards and identify equipment needs or environmental adaptations.
• Document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
