Description

You will be an integral member of a multi-disciplinary health care team providing skilled nursing and rehabilitative care to clients in their homes. You'll conduct home visits, provide clinical social evaluations, and coordinate community resources to support clients' health and independence.

Requirements

• Master's in Social Work with at least 2 years community-based experience
• Accreditation by Council on Social Work Education
• Ability to read, write, and communicate effectively in English
• Strong interpersonal skills and ability to work independently
• Ability to travel to cases as assigned

Responsibilities

• Making home visits to clients in designated geographic territories
• Provide clinical social evaluations and plan appropriate interventions
• Identify and refer to community resources for client and family needs
• Act as advocate for client and family navigating the community system
• Educate patients and families on community resources for health and independent living
