💼 About This Role

You'll lead the clinical intake and enrollment coordination for a new PACE program, ensuring timely and compliant participant enrollment. You'll supervise intake staff, build relationships with referral sources, and improve intake workflows. This role offers the chance to shape a transformative care model for seniors.

🎯 What You'll Do

• Supervise and develop intake coordinator(s) and support staff
• Triage and track referrals, conduct initial clinical screenings
• Coordinate pre-enrollment assessments and home visits
• Maintain intake documentation in EHR/CRM per compliance requirements

📋 Requirements

• Active California RN license in good standing
• 2+ years of clinical nursing experience
• 1+ year of staff leadership experience
• Valid driver's license and ability to travel for home visits

✨ Nice to Have

• Experience with PACE or managed care
• Experience working in interdisciplinary teams
• Bilingual capacity relevant to patient population
