Description

As a Registered Nurse Case Manager in Home Health, you will conduct initial patient assessments, develop and revise care plans, administer medications and treatments, and coordinate care with physicians and community resources. You'll provide personalized, faith-based care to elderly patients in their homes across the greater Lebanon, Ohio area, ensuring high-quality clinical services and effective discharge planning.

Requirements

• Graduate of approved nursing program
• Valid Ohio RN license and current CPR certification
• Minimum 1 year clinical nursing experience
• Valid driver's license, clean driving record, and automobile insurance
• Home Health or Community Health/Hospice experience preferred

Responsibilities

• Completes initial assessment of patient and family to determine home care needs
• Initiates and revises plan of care based on nursing diagnosis and patient status changes
• Administers medications and treatments as prescribed by physician
• Provides health care instructions and counseling to patient and family
• Participates in on-call duties as defined by on-call policy
