💼 About This Role

You'll join Senior Doc, part of Brigade Health, providing mobile primary care to seniors in homes and assisted living communities. You'll manage chronic conditions and build lasting relationships with older adults, supported by a collaborative care model with psych, palliative, and neurology teams.

🎯 What You'll Do

• Provide mobile primary care visits to seniors
• Manage chronic and acute conditions
• Perform comprehensive assessments and care planning
• Collaborate with caregivers, facilities, and interdisciplinary teams

📋 Requirements

• Active NP or PA license
• 1+ years of primary care, geriatrics, or senior care experience
• Valid driver's license and reliable transportation

• Experience in geriatrics, internal medicine, or family practice
• Background in home health, hospice, or palliative care
• Experience in skilled nursing or assisted living
