Description

You will provide personal care and comfort to hospice patients under the supervision of a registered nurse, observing and reporting changes, and documenting care to ensure quality and continuity. This role is crucial in supporting patients and families during their end-of-life journey.

Responsibilities

• Perform personal care needs after patient assignment and attend to requests promptly
• Report inability to perform a task to the Case Manager immediately
