Description

You will provide skilled nursing care to home health patients in the greater Lebanon, Ohio area, working full-time Monday through Friday with some on-call rotation. This role is part of a faith-based ministry focused on person-centered elder care.

Requirements

• Graduate of approved nursing program
• Valid Ohio LPN license
• CPR certification
• Minimum 2 years direct patient care in hospice or home care
• Valid driver's license, auto insurance, and clean driving record

Responsibilities

• Provide skilled professional nursing care to patients as prescribed by physician
• Comply with Medicare Conditions of Participation and Ohio Home Health laws
• Serve as primary caregiver for skilled home health patients
• Educate patients and families on care plans
