Description

You will provide skilled physical therapy to adult and geriatric patients in their homes, helping them regain independence through individualized care plans, therapeutic exercises, and patient education. As part of a multidisciplinary team, you'll assess patient needs, implement treatments, and document progress using a tablet.

Requirements

• Current license as a Physical Therapist in Massachusetts

Responsibilities

• Perform diagnostic tests and measurements of mobility, range of motion, strength, balance, and safety
• Develop and implement individualized care plans including therapeutic exercises, gait training, and balance interventions
• Continually assess and revise PT care plan; participate in multidisciplinary care planning
• Educate clients and family members on rehabilitative care and activities for health, safety, and independence
• Accurately document observations, interventions, and evaluations using a touch pad tablet
