Description

As a Care Navigator at Charlie Health, you'll be the first point of contact for patients and families seeking mental health treatment. You'll manage admissions, ensure a seamless intake process, and collaborate with internal teams to help expand access to life-saving care.

Requirements

• Bachelor's degree in health sciences, communications, psychology, social work, or related field
• 1-2 years relevant work experience in healthcare patient-facing roles
• Metrics- and results-oriented mindset with experience meeting KPIs
• Excellent written and verbal communication skills
• Proficient in Salesforce and Google Suite/MS Office

Responsibilities

• Ensure supportive, positive experience for clients and referral sources
• Work with clients, families, and referral sources to understand needs and preferences
• Make accurate and timely outbound referrals for those not admitted
• Document all interactions in electronic record system
• Collaborate with Clinical Outreach and Partnerships teams
