You'll lead a team providing assistive care to geriatric clients in their homes, conducting assessments and coordinating client care. 🎯 What You'll Do

• Visit geriatric clients and conduct assessments
• Develop and execute client care plans
• Coordinate communications between clients, field staff, and clinicians
• Supervise and support Home Health Aide staff

📋 Requirements

• Registered Nurse (RN) license
• 2 or more years of experience as an RN
• Home Care experience preferred
• Supervisory experience as a Nurse Manager preferred
