Description

You will provide 1:1 home health visits to clients in Walpole, NH and surrounding areas, following care plans, administering medications, performing wound care, and monitoring conditions. This role offers flexible schedules, great work/life balance, and opportunities for career advancement.

Requirements

• Current Registered Nurse license in New Hampshire
• Minimum one year of recent verifiable clinical (medical/surgical) experience
• Graduation from an accredited nursing program
• Ability to work independently and manage time effectively
• Solid computer skills; prior experience with EMR preferred

Responsibilities

• Follow designated care plans in accordance with patient needs
• Make home visits and perform assigned duties including medication administration, wound care, treatments, and procedures
• Monitor client conditions and report changes to Clinical or Client Services Manager
• Perform client assessments and case management
• Accurately document observations, interventions, and evaluations using electronic tablet
