Description

As a home health RN, you will perform home visits with adult and senior patients, providing skilled nursing and rehabilitative care to help them remain at home. You'll use your assessment skills to identify changes in patient condition, manage urgent clinical situations, and document care efficiently.

Requirements

• Current/recent experience in Medicare Home Health with OASIS documentation required
• Ability to work independently and make real-time clinical decisions
• Strong problem-solving skills in unpredictable home environments
• Comfortable with point-of-care documentation and technology
• Flexible and adaptable to changing patient needs and schedules

Responsibilities

• Use head-to-toe assessment skills to identify subtle changes in patient condition
• Provide acute and post-acute care (CHF, COPD, wound care, IV therapy, post-surgical care)
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation, teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation
