💼 About This Role

You'll act as the primary point of contact for clients with complex needs, coordinating care with doctors, specialists, and social services. You'll develop care plans and advocate for members to ensure access to appropriate services. This hybrid role requires frequent travel in Los Angeles County.

🎯 What You'll Do

• Assess member needs in physical health, mental health, social supports, etc.
• Develop and oversee client care plans and goal settings.
• Conduct outreach and engagement activities for ECM program linkage.
• Complete documentation and maintain patient health records in EMR.

📋 Requirements

• Active, unrestricted LCSW, LMFT, or LPCC license in California required.
• 1+ year of experience as a care manager or community health worker supporting vulnerable populations.
• Willing to work Monday-Friday 8:30am-5:00pm with flexibility for evenings/weekends.
• Clean driving record, valid driver's license, and reliable transportation.
