Description

You will provide 1:1 in-home physical therapy to adult and geriatric clients, performing assessments, developing care plans, and educating clients and families. This role offers flexible scheduling and a $5k sign-on bonus for full-time day-time hours.

Requirements

• Experienced Physical Therapist (PT) license
• Ability to provide home health visits to adult and geriatric clients
• Competence in performing physical therapy assessments and interventions
• Proficiency in documenting care using a touch pad tablet
• Availability for flexible scheduling (full-time, part-time, or per diem)

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements (mobility, range of motion, strength, balance)
• Implement wound prevention and care using evidence-based dressings
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Educate clients and families on rehabilitative care and activities for independent living
