💼 About This Role

You'll provide rehabilitative care to adult and geriatric clients in their homes as part of a multi-disciplinary team. You'll make home visits, develop care plans, and educate clients and families on independent living. This role offers flexible scheduling and career advancement opportunities.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, and balance.
• Develop and implement individualized care plans with manual therapy and exercises.
• Document observations and interventions using an electronic tablet.

📋 Requirements

• Current AZ Physical Therapist license.
• Minimum 1 year of recent work experience as a Physical Therapist.
• Graduation from an accredited PT program (APTA or equivalent).
