Description

As a PRN Registered Nurse Case Manager at Luminary Hospice in Carmel, Indiana, you will provide compassionate hospice care to patients and their families in their homes and communities. You will perform initial and ongoing assessments, develop and implement care plans, administer medications, and coordinate with the interdisciplinary team to ensure high-quality end-of-life care.

Requirements

• Experience in nursing with emphasis on community health education/experience
• Ability to perform comprehensive physical assessments and develop nursing diagnoses
• Strong communication skills for patient education and team coordination
• Ability to work on-call duties as defined by policy
• Commitment to continuing education and quality improvement

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family needs
• Develop, implement, and revise individualized care plans in collaboration with patient, family, and IDG
• Administer medications and treatments as prescribed and provide health counseling
• Document patient condition and care, and communicate changes to physician and team
• Coordinate care and supervise ancillary personnel as needed
