Description

You will provide occupational therapy to adult and geriatric patients in senior living communities, performing evaluations, developing care plans, and working with a multi-disciplinary team to help patients remain independent at home.

Requirements

• Current NJ Occupational Therapist license
• Minimum one year recent OT experience
• Graduation from accredited OT program
• Ability to work independently and manage time
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Perform outpatient therapy evaluations and develop care plans
• Operate at top of licensure using evidence-based approaches
• Collaborate with therapy partners for care transitions
• Educate clients, family, and caregivers on care plans
• Complete evaluations, treatments, and discharge planning using EMR
