You'll join Vynca as a Clinical Lead Care Manager, coordinating care for clients with complex needs. Your core impact will be managing client cases and ensuring seamless communication across medical and social providers. This role stands out for its hybrid work model in Imperial County.

🎯 What You'll Do

• Assess member needs across physical, mental, and social health.
• Develop and oversee client care plans and goal setting.
• Conduct outreach and engagement for ECM program linkage.
• Maintain up-to-date patient health records in EMR system.

📋 Requirements

• Active California LVN license.
• 2+ years as a care manager or community health worker.
• Working knowledge of government and community resources.
• Clean driving record, valid driver's license, and reliable transportation.

✨ Nice to Have

• Experience with motivational interviewing and trauma-informed care.
• Bilingual in Spanish.
• Experience with electronic medical record systems.
