💼 About This Role

You'll deliver comprehensive medical care to residents in skilled nursing and senior living settings, managing complex conditions independently while collaborating with an interdisciplinary team. Your work will directly improve outcomes, reduce hospitalizations, and ensure compassionate, consistent care for vulnerable populations. Clinical autonomy and patient-centered decision-making are core to this role.

🎯 What You'll Do

• Provide primary, urgent, and preventive care to residents.
• Independently evaluate, diagnose, and treat acute and chronic conditions.
• Conduct initial admissions and follow-up assessments.
• Collaborate with facility Medical Directors and interdisciplinary teams.

📋 Requirements

• Active, unrestricted Nurse Practitioner license in applicable state(s).
• Current national NP certification (ANCC, AANP, or equivalent).
• Clinical training and experience in adult, family, or geriatric medicine.

✨ Nice to Have

• Experience in internal medicine, geriatrics, cardiology, or post-acute care.
• Active DEA registration or ability to obtain.
• Prior experience in SNF, LTC, ICU, or Emergency Department settings.
