You'll join a mission-driven care team, meeting patients in the community to provide social support, advocacy, and navigation. You'll conduct needs assessments, connect patients with resources, and help them achieve health goals. This role offers comprehensive training and a supportive multidisciplinary team.

🎯 What You'll Do

• Meet patients in community, home, or healthcare facilities to conduct needs assessments.
• Help patients with health-related social issues like homelessness, substance use, and hunger.
• Create and maintain a directory of community resources (food banks, housing, etc.).
• Document patient encounters accurately in technology systems.

📋 Requirements

• Highly organized and self-motivated to work independently.
• Ability to work with diverse communities in an empathetic manner.
• Computer literate with experience using technology for virtual communication.
• Reside within a commutable distance of East Cleveland.

✨ Nice to Have

• Community Health Worker certification.
• Long time resident of the Greater Cleveland area and knowledgeable of community resources.
• Experience conducting home visits and outreach.
