💼 About This Role

You'll provide skilled physical therapy services in patients' homes, evaluating and treating to restore function and independence. You'll develop care plans and coordinate with physicians and PTAs, focusing on high-quality outcomes for a leading home health agency. Concierge administrative support and zip code-based territories minimize travel and maximize clinical time.

🎯 What You'll Do

• Perform in-home evaluations and develop care plans
• Help patients with mobility and regain independence
• Create detailed treatment plans for each patient
• Coordinate with physicians and report patient progress

📋 Requirements

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
