You'll provide direct patient care using Circadia's AI platform that predicts adverse events days in advance. Your work will reduce preventable hospitalizations by acting on real-time health signals and coaching facility teams. You'll blend bedside care with tech-to-bedside integration across skilled nursing facilities.

🎯 What You'll Do

• Conduct initial and follow-up assessments on flagged residents
• Train and coach staff on using Circadia in clinical workflows
• Interpret trend-based insights to prioritize high-risk patients
• Complete documentation within the same day using AI-powered tools

📋 Requirements

• Certified and licensed Nurse Practitioner in PA
• 2+ years NP experience with complex adult populations
• Valid PA driver's license and comfortable traveling locally
• Eligible for Medicare participation

✨ Nice to Have

• Tech-savvy with Google Suite, multiple EMRs, Slack, AI tools
• Spanish language preferred
