💼 About This Role

You'll join the largest home health agency in San Francisco as an entry-level Field RN, providing hands-on patient care in the Auburn, Colfax area. Under the guidance of a Case Manager RN, you'll develop fundamental home health nursing skills while contributing to our industry-leading <10% hospital readmission rate.

🎯 What You'll Do

• Provide hands-on patient care including wound care, tube management, and IV therapy
• Implement and document plan of care under Case Manager RN supervision
• Educate patients and families on diagnosis and self-care techniques
• Participate in weekly case conferences and on-call weekend rotation

📋 Requirements

• One year of recent RN or LVN experience
• Current California RN license
• Valid CA driver's license with acceptable record
• Current Basic Life Support (BLS) card
