You'll join a dynamic team at Vynca, a mission-driven organization focused on transforming care for individuals with complex needs. You'll provide clinical oversight and supervise a team of ECM staff to address medical, behavioral, and social determinants of health. This role offers the chance to work remotely while making a profound impact on patient lives.

🎯 What You'll Do

• Supervise a team of 10-15 direct reports for high-quality care management.
• Oversee client outreach, enrollments, and Individual Care Plans.
• Drive the team to meet key performance indicators (KPIs).
• Provide clinical oversight and direct care management as needed.

📋 Requirements

• Active, unrestricted LCSW or LMFT license in California required.
• 2 years of people management experience.
• 2-3 years of care management experience.
• 1-2 years of post-acute experience.

✨ Nice to Have

• 4 years of people management experience preferred.
• Salesforce experience is a plus.
• Working knowledge of lean Six Sigma methodology preferred.
