Join Bold's clinical team to deliver high-quality telehealth care to older adults, focusing on preventive and lifestyle medicine. You will provide personalized care plans, use EMR for documentation, and collaborate with care coordinators and health coaches.

Requirements

• Active and unrestricted NP licenses in CA, TX, and/or PA
• 5+ years clinical experience
• 2+ years telehealth experience with high EMR proficiency
• Strong foundation in preventive and lifestyle-focused care including weight, hypertension, diabetes management
• Comfortable working independently in a fast-paced startup environment

Responsibilities

• Deliver telehealth care to older adults with a focus on prevention, chronic disease management, and lifestyle interventions
• Provide feedback on clinical protocols, documentation standards, and workflows
• Use EMR for patient documentation and communication with other providers
• Collaborate with care coordinators, health coaches, and clinicians to develop personalized care plans
