💼 About This Role

You'll serve as Waymark's frontline presence in the community, connecting Medicaid patients with social support and resources. Your work directly improves health outcomes by addressing barriers like homelessness and hunger. This role offers a multidisciplinary team and a paid training program.

🎯 What You'll Do

• Conduct needs assessments and help patients set health goals.
• Create and maintain a directory of community resources.
• Manage relationships with healthcare providers and care team.
• Document patient encounters in technology systems.
• Accompany patients to medical appointments as needed.

📋 Requirements

• Driver's license and access to insured vehicle.
• Reside within commutable distance of Lake County, Illinois.
• Availability for set schedule including one weekend day and one late shift per week.
• Computer literacy for virtual communication and documentation.
