🎯 What You'll Do

• Complete initial and comprehensive patient assessments for hospice needs.
• Administer medications and treatments as prescribed by physician.
• Coordinate patient care and support case managers after hours.
• Maintain accurate clinical notes and communicate with physicians.

📋 Requirements

• Bachelor's or Associate's degree in nursing or related field.
• Active nursing license in good standing in Ohio.
• 2-5 years experience in hospice, palliative care, or healthcare management.
• CPR certification.

• 1+ year experience with Medicare Hospice Certification and CHAP guidelines.
• Proficiency in MS Office and clinical software.
