You will provide 1:1 home health visits to clients in Luzerne County, following a designated care plan and performing duties such as medication administration, wound care, and client assessments. Requirements

• Current Registered Nurse license in Pennsylvania
• Minimum one year of recent clinical medical/surgical experience
• Graduation from an accredited nursing program
• Ability to work independently and manage time effectively
• Solid computer skills; experience with electronic medical records preferred

Responsibilities

• Follow designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform duties including medication administration, wound care, treatments, and procedures
• Monitor clients' conditions and report changes to Clinical or Client Services Manager
• Accurately document observations, interventions, and evaluations using a touch pad tablet
