💼 About This Role

You'll provide skilled in-home physical therapy to help patients regain independence and function. Your care plans and coordination with physicians will directly reduce hospital readmissions. 🎯 What You'll Do

• Perform in-home evaluations and develop care plans.
• Help patients with mobility and regain independence.
• Coordinate with physicians to ensure quality care.
• Supervise Physical Therapy Assistants (PTAs).

📋 Requirements

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
