Description

You will provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes, performing evaluations, developing intervention plans, and educating patients and families to promote independent living.

Requirements

• Current Maryland Occupational Therapist license
• Minimum 1 year recent work experience as an Occupational Therapist
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time effectively
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Perform thorough evaluations of functional status including muscle function and ADL performance
• Develop individualized intervention plans to achieve favorable outcomes
• Evaluate home environment for hazards and identify equipment needs
• Educate patients and family members on rehabilitative care and safety
• Accurately document observations and interventions using a touch pad tablet
