Description

As a home care Speech Language Pathologist, you will provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes, performing evaluations, developing treatment plans, and educating patients and families.

Requirements

• Current license as a Speech Language Pathologist in North Carolina
• Certificate of clinical competence in speech from ASHA or equivalent
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform evaluations to assess and diagnose speech, language, communication, or swallowing difficulties
• Develop treatment plans based on evaluation, physician's orders, and goals
• Educate patients and family members on rehabilitative care
• Document observations and interventions using a touch pad tablet
