Description

You will join a multi-disciplinary health care team providing rehabilitative care and skilled nursing to adult and geriatric clients in senior living communities. Make home visits, perform diagnostics, develop care plans, and educate clients to promote health and independence.

Requirements

• One year prior clinical experience as a licensed Physical Therapist
• Valid Physical Therapist license
• Experience with adult and geriatric clients
• Ability to travel to senior living communities in Lancaster and York counties

Responsibilities

• Perform diagnostic tests and measurements such as joint mobility, gait, balance, and safety assessments
• Develop and implement individualized care plans including manual therapeutic exercises and gait training
• Implement integumentary protective interventions for wound prevention and repair
• Educate clients and family members on rehabilitative care and activities
• Accurately document observations and interventions using a touch pad tablet
