Description

You will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, helping them remain independent. As part of a multi-disciplinary team, you'll develop care plans, perform diagnostic tests, and educate clients and families.

Requirements

• Current Maryland Physical Therapist license
• Minimum one year recent experience as a Physical Therapist
• Graduation from an APTA or CAAHEP accredited program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements of mobility, range of motion, strength, balance, and safety
• Implement wound prevention and repair interventions using evidence-based dressings
• Develop and adjust individualized care plans including manual therapy, gait training, and balance exercises
• Educate clients and family members on rehabilitative care and activities for independence
